TATA CARA PENDAFTARAN BAKAL CALON PEMBANTU DEKAN FAKULTAS TEKNIK UNIVERSITAS DIPONEGORO PERIODE TAHUN 2015 – 2019

  1. Pendaftaran Bakal Calon Pembantu Dekan dilakukan secara langsung ke Kepala Bagian Tata Usaha Dekanat Fakultas Teknik Undip Semarang tanggal 18 – 29 Mei 2015.
  2. Berkas pencalonan dimasukkan ke dalam stopmap/amplop tertutup disusun berdasarkan urutan sebagai berikut :
  3. Pas photo berwarna ukuran 4×6 cm sebanyak 2 lembar;
  4. Formulir Kesediaan (Form 1) ;
  5. Daftar Riwayat Hidup, yang mencakup: Data Pribadi, Riwayat Pendidikan Formal, Riwayat Tugas Tambahan, Riwayat Tugas Tambahan Lain, Riwayat Jabatan Fungsional, Karya Ilmiah /Publikasi dan Penghargaan (Form 2);
  6. Surat keterangan sehat dari dokter pemerintah (RS Dr Kariadi / Puskesmas atau RSUD Kota Semarang).
  7. Setiap Bakal Calon Pembantu Dekan yang telah mendaftar akan mendapatkan tanda terima bukti pendaftaran yang di dalamnya memuat nomor urut, hari, tanggal, dan jam pendaftaran, serta kelengkapan berkas pendaftaran.
