  1. Surat Keterangan & Permohonan (https://ft.undip.ac.id/download/ Nomor 11, poin 2)
  2. Surat bukti /dokumen resmi sesuai kejadian yang dialami
  3. Surat Pernyataan tidak sedang menunggu hasil ujian masuk perguruan tinggi lain (bit.ly/SuketTidakSedangMenungguHasilUjianPerguruanTinggiLain)
  4. Fotocopy Kartu Keluarga
  5. Fotocopy rekening listrik
  6. Essay keadaan ekonomi (selengkap-lengkapnya, detail, format bebas)
  7. Pengajuan berkas dengan melakukan pengisian pada form berikut: bit.ly/PengajuanBandingUKTCamabaSBMPTN2021 dengan format file: Nama_Jurusan (dijadikan 1 file PDF)
